Key Responsibilities:
* Evaluate medication orders to ensure the appropriateness of the drug, dosage, route of administration, and potential drug interactions.
* Pay special attention to geriatric considerations to ensure the safety and efficacy of medications for elderly patients.
* Collaborate with healthcare providers to optimize patient care and medication management.
Qualifications:
* A bachelor's degree from an accredited School of Pharmacy or a Doctor of Pharmacy (PharmD) degree.
* A current, unrestricted Texas Pharmacist License.
Preferred Experience:
* Experience in long-term care settings.
* Familiarity with Framework LTC software is a plus, but not required.
